Community Connections Team

Community Connections Team (CCT) is a program within the Office of Health Equity, Diversity & Inclusion (HEDI). Marshfield Clinic Health System (MCHS) Community Connections Team (CCT), the University of Wisconsin (UW) System and community organizations collaborate to address health inequities in our communities. MCHS screens for social needs such as food, utilities, housing and transportation. MCHS has a partnership with the UW System to recruit and train CCT Navigators (volunteers). CCT Navigators help connect patients to resources in the community to help address their needs. MCHS staff, AmeriCorps members, CCT Navigators and patients can find resources utilizing a social care platform called Caring for Communities by findhelp - Search and Connect to Social Care.

 

Marshfield Clinic Health System strives to address the social factors that impact our health. We also are preparing our future workforce to understand the impact of these factors while supporting providers in addressing the social needs of our patients.
